Home Nutrition Support Program

Soleo Health's Nutrition Support Program provides exceptional, personalized service and nutrition care expertise to patients requiring total parenteral nutrition (TPN) at home or in the long term care setting.

An Interdisciplinary Team Approach
Soleo Health uses an interdisciplinary team approach when managing nutrition support patients. Our nutrition program has been designed by experienced clinicians and adheres to the home infusion standards set by industry organizations such as the American Society for Parenteral and Enteral Nutrition (ASPEN), National Home Infusion Association (NHIA), American Society of Health-System Pharmacists (ASHP) and Infusion Nurses Society (INS).

Pharmacists
Soleo's pharmacy team communicates with physicians to ensure each patient is treated effectively and efficiently. Experienced pharmacists compound TPN in a sterile environment (as per USP 797 pharmacy compounding standards), monitor for drug interactions, and offer administration and medication counseling. Our pharmacy team is available 24 hours a day, 7 days a week.

Registered Nurses
Soleo’s team of registered nurses (RNs) is highly experienced in home nutrition therapy support. Each nurse conducts ongoing patient assessments to help prevent avoidable calls and visits to the physician’s office or emergency department. They provide extensive patient and caregiver education, and implement central access device maintenance and monitoring to prevent catheter complications. Our nursing team is available 24 hours a day, 7 days a week.

Registered Dietitians
Soleo’s registered dietitians are Certified Nutrition Support Clinicians (CNSC). A CNSC is specifically trained in the management of nutrition support and will assist with monitoring TPN in the home. Their expertise helps ensure every patient infusing TPN receives the appropriate TPN formulation based on individual assessment (from fluids, calories and protein, to electrolytes and micronutrients).

Reimbursement Specialists
Soleo’s reimbursement specialists are trained to advocate and provide assistance with insurance benefit verification and authorization, and an explanation of any financial responsibility. They are also experienced in the requirements of Medicare Part B coverage of home TPN and will quickly review medical records to determine if Medicare criteria can be met.

National Recognition
Soleo Health’s Certified Nutrition Support Clinicians had the honor of presenting a clinical poster at ASPEN’s Clinical Nutrition Week 2016. The poster highlights Soleo’s expertise in the qualification of home TPN coverage under Medicare Part B guidelines.

Indications for Home TPN can include:

  • Short bowel syndrome
  • Small bowel obstruction
  • Severe Crohn's disease
  • Severe pancreatitis
  • Intestinal fistulas
  • Malabsorption
  • Gastroparesis/motility disorders
  • Cancer cachexia
  • Severe hyperemesis gravidarum
  • Complications of bariatric surgery
  • Pediatric GI diagnoses

Each of these clinical conditions may result in unnecessary hospitalization and/or emergency department visits. Soleo Health provides an easy and patient-friendly alternative to hospitalization by offering the patient and family choice in the type of care they receive and who provides it.

Home Start TPN Program
Soleo's Certified Nutrition Support Clinicians offer the expertise to initiate TPN in the home setting, advocating for hospital avoidance when clinically appropriate. Our clinical team will assess patient appropriateness, considering the risk vs benefit of initiating care in the home.
