PHARMACIST - San Francisco, CA

We are currently seeking a Full Time Pharmacist for our San Francisco market who will prepare pharmaceutical prescriptions in an accurate and timely manner and provide related distributive and clinical pharmacy activities to provide services to specialty infusion therapy patients.  The ideal candidate will possess the ability to work effectively with others to provide safe and effective drug therapy.  The ability to problem solve and supervise technical and other support staff, excellent verbal and written communication skills, as well as organizational and time management skills are a must.


  • Coordinates and facilitates new referrals/admissions through collaboration with nursing, reimbursement, operations and sterile products
  • Reviews, receives/verifies all new patient orders and documents the same on the respective order form
  • Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy
  • Conducts initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problem or needs
  • Conducts and revises the patient plan of care
  • Assists the physician and nursing staff in setting therapeutic objectives for drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
  • Monitors the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
  • Communicates the results of therapeutic drug monitoring activities to the physician, nursing and other appropriate persons, makes appropriate recommendations which are designed to enhance the outcomes associated with drug therapy.
  • Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
  • Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians and other healthcare professionals.
  • Prepares sterile admixtures, dispenses all pharmaceuticals and related supplies and equipment in accordance with federal, state and local laws and standards of practice which governs the profession of pharmacy.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Programs infusion control devices as indicated
  • Participates in departmental QA activities
  • Attends designated department meetings as scheduled
  • Adheres to all Company Policies, Procedures, and Standards of Conduct covered during initial orientation and referenced in branch handbooks, guides and manuals
  • Attends mandatory annual inservices that are required by current position held

  • Bachelor of Science in Pharmacy required
  • Doctor of Pharmacy (Pharm D) or MS in Pharmacy is preferred
  • Current applicable state licensure as a registered Pharmacist
  • Minimum 2 years’ Pharmacist experience in an acute care setting or 1 year recent experience in home infusion therapy
  • Experience in and strong knowledge of IV admixture services and IV drug therapy
  • Efficiency in CPR Plus is desired
  • Excellent customer service skills
  • Capability of assuming responsibility for the evaluation and monitoring of drug therapy relative to the total needs of the home care patient in collaboration with doctors, nurses and other healthcare professionals
  • Proficient with Microsoft Excel, Word and Outlook.
