The Soleo Direct program assists hospitals, health systems, physicians, health plans and payors in identifying appropriate patients eligible for receiving treatment in alternate sites of care through multidisciplinary collaboration, as they are discharged. Soleo Direct utilizes proprietary algorithms to determine targeted diagnosis-related groups (DRGs) and establishes care paths for specific diagnoses that can safely be treated with intravenous or injectable medications in the home or alternate site setting.
Soleo Direct specifically customizes care paths using these clinical algorithms for hospital systems to help facilitate the discharge planning process while also collecting reportable data metrics to provide deeper intelligence. It has the ability to track readmissions in both outpatient and transitions of care settings, resulting in evidence-based data that helps better manage patient care plans utilizing Soleo Health’s proprietary clinical outcomes program, SoleMetrics®.
In a recent study of cellulitis patients conducted over a one-year period, Soleo Health used Soleo Direct to deliver clinical interventions, which enabled the transition of patients to lower cost of care sites, directly bypassing emergency departments and hospitalizations. The study resulted in cost savings of nearly $500,000 as revealed by data provided through SoleMetrics.
