Soleo will customize a health system-specific strategy by agreeing to meet quality standards, share data and collaborate with administration, nursing, case management, social work, physician and provider teams.

The Soleo care team will define metrics for patient transition and coordination of care; manage and measure outcomes. We ensure a seamless transition from the hospital, emergency department or urgent care to the patient’s home or one of our ambulatory infusion centers.

Soleo Care Transitions is a collaborative site of care optimization program. Soleo works with payors in initiating programs to help identify members on high-cost medications and assists in selecting the most clinically appropriate and cost-effective setting. Emergency Department Triage

To improve efficiency and increase throughput capacity, Soleo Direct utilizes a triage algorithm for targeted diagnosis-related groups (DRGs) to establish care paths for specific diagnoses that can safely be treated with intravenous medications in the home or at alternative sites.
