Your care journey with us starts when your physician provides a referral. Our team then reviews your health insurance coverage and develops a personalized treatment plan tailored to your needs.
Once we receive your referral, our team will review your insurance benefits, guide you through the onboarding process, and discuss how our specialized services can meet your unique health needs.
As you start treatment, you will connect with a Patient Ambassador who will be your primary point of contact, answer all your questions, and transition you to the clinical care team who will treat you.
At Soleo Health, we build and maintain relationships with major pharma companies to receive cutting-edge drugs and medications to treat complex, rare and ultra-rare health conditions for our patients.
We provide medication management support, from assistance with side effects to perfecting the dosage to refill reminders.
For those with conditions that require clinical monitoring and interventions, our personalized care and treatment plans are exceptional.
For patients who aren’t nearby or are unable to visit our infusion centers, we also offer home infusion therapy services. A registered nurse will visit and provide the treatment you need in the comfort and convenience of your home. We can also teach you how to administer home infusion therapy yourself.
