Every new patient joins Soleo Health Milwaukee with a patient referral from their usual physician.
The process is straightforward. Simply ask your provider for a referral, and they will complete the paperwork and send it to Soleo Health. We’ll then contact you to discuss your needs, insurance benefits, and eligibility.
As part of the onboarding process, we match you with a Patient Ambassador. Your ambassador will get to know you and will assist with your insurance benefits investigation. Once you start treatment with us, a consistent clinical team will take care of you through the remainder of your journey.
