Your Patient Journey
Every new patient joins Soleo Health Milwaukee with a patient referral from their usual physician.
The process is straightforward. Simply ask your provider for a referral, and they will complete the paperwork and send it to Soleo Health. We’ll then contact you to discuss your needs, insurance benefits, and eligibility.
As part of the onboarding process, we match you with a Patient Ambassador. Your ambassador will get to know you and will assist with your insurance benefits investigation. Once you start treatment with us, a consistent clinical team will take care of you through the remainder of your journey.
Referrals Process
To join the Soleo Health family, you need a referral from your provider. We’ve made this process simple to streamline the onboarding process:
- Advise your provider that you wish to utilize Soleo Health.
- They complete the online referral paperwork.
- We will contact you to discuss insurance verification and onboarding.
Once these initial steps are complete, you can begin treatment through Soleo Health Milwaukee.
